2011 Motorcycle / Classic Auto Poker Run Registration Form

 

• Motorcycles and Autos of all makes and models welcome

• Event will be held on May 14, 2011 at Machinists Hall

• 212 Utz Lane, Hazelwood, Missouri 63042

• Registration begins at 7 a.m. - the ride will depart at 8:30 a.m. sharp

• Lunch will be served at the half way point

• Ride will return to IAM Hall for dinner at approximately 5 p.m.

 

• Dinner will be served 5:00 p.m. to 7:00 p.m. at Machinists Hall above

 

• All entries / vehicles  must include the $35 for rider / driver

 • All entries / vehicles  must include the $35 for each passenger

• All entries / vehicles must raise $50 in pledges for Guide Dogs of America

• Poker hands $5 for one or 3 hands for $10 - no limit on number purchased

Pledges must be paid day of ride to be eligible for prizes.

I hereby understand that there are risks associated with participation in this Run, and accept that I am solely responsible for myself, my passengers, my vehicle and its contents.  I agree to release the International Association of Machinists and Aerospace Workers, Guide Dogs of America, District Lodge 837, Local Lodge 822, and all sponsors or officials of the Run from any liability of any kind that results from participation in this event.  This release is from any liability to the undersigned or to any claims on their behalf.  I willingly assume and agree to accept all risks associated with participation in this Run.  I maintain a valid auto liability policy for the vehicle described above, and will abide by all rules and regulations of the show.  I also give permission for the free use of my name and/or photo in any broadcast, telecast, web page, or other account of this event.
